Neurostimulation and neuromodulation show potential for treating dry eye syndrome by modulating nerve and cell activity to restore tear production and maintain eye health.
FREMONT, CA: Artificial tears have been the primary choice for individuals with dry eye disease for a considerable time, although they provide only a limited remedy. Neuromodulation is a promising method for treating dry eye that aligns with the aspect of the disease that involves neurosensory dysfunction. These developing therapies primarily focus on the lacrimal functional unit, which can be utilised to encourage the production of tears. When this is triggered, it causes the production of a full teardrop by activating the lacrimal gland, meibomian gland, and conjunctival goblet cells.

The natural stimulation of tears provides more benefits than using artificial tears. Chemical analysis of natural tears shows that they contain various substances such as antibodies, growth factors, and many other proteins and immune molecules that aid in the healing of the eye's surface. When the trigeminal nerve is dysfunctional or irritated by a disease process, various unpleasant outcomes can occur. The link between the brainstem and the lacrimal functional unit through the trigeminal nerve is constantly active unless there is an intervention that interrupts this connection.
Neuromodulation
Neuromodulation involves targeting particular receptors located on the ocular surface's corneal nerves to produce the desired effect. With the Transient receptor potential cation channel subfamily M (melastatin) member 8 (TRPM8) agonist, AR-15512, the receptor is precisely stimulated.
During a phase 2b study, 121 patients received 0.0014 per cent AR-15512, 122 patients received 0.003 per cent AR-15512, and 126 patients received a vehicle, all administered twice daily for 12 weeks. The signs and symptoms of dry eye disease, including changes in ocular discomfort and the anaesthetised Schirmer's scores, were the primary endpoints.
The use of 0.003 per cent AR-15512 led to a quick and persistent increase in the unanesthetised Schirmer's score, evident on day one and day 14 of treatment, and patients experienced enhancements in ocular surface staining and hyperemia. Furthermore, there were improvements in the evaluation of dry eye symptoms, ocular discomfort, eye dryness, and other quality-of-life aspects.
The phase 3 COMET-3 trial of AR-15512 for the treatment of dry eye disease began last year with the first participant receiving the medication. The primary measure of effectiveness is the unanesthetised Schirmer's score, which evaluates tear production.
According to the phase 2 data, the immediate outcome of TRPM8 stimulation was an increase in tear production. Additionally, TRPM8 stimulation has a mechanism in the central nervous system that reduces pain signals, leading to improved tear production and decreased discomfort.
Instead of relying on the inflammation to subside and hoping for the recovery of the neurosensory aspect of healthy tear production, it is more desirable to manage it with finesse and accuracy. Research on the neuropathic connection between the eye surface and the brain is expected to be a promising area of study in the near future. Some treatments available now have already demonstrated positive results in reducing corneal pain.
Changing the transmission of information through the trigeminal nerve is a highly promising research area. Patients who experienced pure neurogenic pain, which was not alleviated with topical anaesthetics, experienced relief when their trigeminal nerve was stimulated using TrueTear.
Neurostimulation and neuromodulation are emerging as promising approaches for treating dry eye syndrome, a common and often chronic condition that causes discomfort, irritation, and vision problems. These techniques involve the use of electric or chemical signals to modulate the activity of nerves and other cells in the eye and surrounding tissues, to restore normal tear production and maintain eye health.
