Prenatal Cell-Free Screening Results and Maternal Malignancy
Using non-invasive foetal aneuploidy screening with cell-free DNA (cfDNA) in pregnant patients with a malignancy helps to detect circulating cfDNA from the tumour in addition to the usual placental and maternal cfDNA. In a study, pregnant patients were screened for prenatal genome-wide cfDNA screening, and malignancy-suspicious results or multiple chromosome imbalances led to a new diagnosis of malignancy in a few patients. As the results do not correlate with the tissue of origin of malignancy, the proper clinical evaluation of malignancy-suspicious prenatal cfDNA results is unclear. Therefore, patients undergoing prenatal foetal aneuploidy screening with a cfDNA test must be aware of the possibility of this rare incidental finding.
Screening Reproductive-Age Females for Chagas Disease
People with Chagas disease are often unaware of their infection and its effects, like maternal cardiovascular and gastrointestinal complications, transmission to the foetus, and hydrops fetalis. As a result, professionals recommend screening for Chagas disease in females of reproductive age. As antitrypanosomal drugs prevent congenital infection, screening is usually performed before pregnancy but contraindicated during pregnancy. Although pre-pregnancy screening is not performed, testing in pregnancy is useful because early diagnosis and treatment of infected infants improve the prognosis of congenital Chagas disease. Moreover, maternal treatment can be initiated postpartum if they are appropriate.
Self-Monitoring of Blood Pressure: Not Better for Early Detection of Preeclampsia
It has been suggested that daily blood pressure monitoring could help detect preeclampsia early. However, self-monitoring with telemetry support did not result in earlier office-based detection of hypertension or a lower incidence of severe hypertension or preeclampsia in two significant randomised trials conducted comparing self-monitoring of blood pressure and usual care in pregnant patients at high risk for developing preeclampsia. Most of the discrepancies between home and office readings among the participants were attributed to white coat hypertension, even though these findings indicate the safety of usual care. For some patients, self-monitoring may help prevent needless escalation of examination and treatment.
