An All-Encompassing Ventilator Platform

Non-Invasive ventilation has been a standard of care in the acute care environment for many years. For many clinicians, a major challenge is that they usually need a different device to apply proper NIV. Although many ICU ventilators offer an NIV option on their ventilators, the performance is often not equal to that of the stand-alone NIV device.

There are certain performance characteristics that are essential in a device that is intended for NIV use. These include accurate trigger detection and fast trigger response times in the face of varying leaks, the ability to work with intentional leaks (vented mask or intentional leak circuits), comprehensive monitoring, and fast pressurization.

Simply put, as leak increases, degradation of trigger detection, trigger response time, and incorrect breath cycling occurs. Nowhere was this more clearly demonstrated than during the treatment of COVID 19 patients this past year. As cases of pandemic-related respiratory failure and experience caring for them increased, it was clear that maximizing non-invasive support through HFOT and NIV was the ideal clinical pathway. For the COVID 19 patient, to avoid intubation, NIV support was continued on patients that 18 months ago would have been intubated.

These patients were breathing at very high rates, with high pressures, high FiO2 requirements and periods where their peak inspiratory flowrates reached the maximum capabilities of most ICU ventilators. Combining these performance needs with the ability to maintain synchrony in the presence of varying high leaks calls for NIV performance expectations uncommon in today’s global ventilation market,” says Brent Chamblee, CEO of Origin Medical Devices.
